Upon scheduling your appointment you will receive an account entry link to "Elation EHR" sent to your Hawkeye 电子邮件. Please complete the following forms before your first visit:

健康 Sciences 学生 Pre-Appointment Forms

Forms for your Preparticipation Physical Evaluation will be sent to you electronically after your appointment at the 学生健康诊所 is scheduled. Please complete your forms prior to your appointment.
If you will be going to an off-campus clinic for your physical, print the following forms and take them with you to your physical:
All RedTail athletes will have an 电子健康记录. Physicals scheduled and performed in the 学生健康诊所 will be entered in the student athlete's 电子健康记录. A printed version can be shared by the patient or at the patient's designation.
